Would be hard to build it??
The D-pad on the SMS is just awful for Games like Mortal Kombat 2

Can't really answer your question but as many members here will testify (and some wont ) the Honeybee controller is the way to go. Very similar D-pad to the Nintendo one and they pop on on ebay all the time (at least here in Australia they do)

The NES pad is +-shaped and the HoneyBee one is circular with a raised +. I'd say the NES one is more comfortable.
To mod, all you need is a soldering iron and a bit of brute force. Remove the encoder chip and cable from the NES pad, then attach the cable from an SMS pad (or any 9-pin joystick) to the right buttons - there's one wire per button and one common wire. But it would be heretical to actually use it.

When I've modded 6-buttons to work on Vectrex and SMS (I know the 6-button works on SMS already, but there's that whole deal with the few games that freak out with Gen controllers) it makes it a lot easier to do a clean job.

I fired up my SMS for the first time in a couple of years last night and I noticed the inadequacy of the D-pad. I solved the issue by using a homebrew/third party pad I bought on eBay that I use for the Amiga. That controller is built in the Saturn style and the D-pad has the suspension you find on the later Saturn controllers.
I bought it a few years ago and I couldn't give you the name of the seller I got it from but that's the gamepad I would recommend.

You can also just use a Mega Drive controller.
I know it's not compatible with all games but it's only a small minority.

Its a very easy fix to get any Mega Drive controller to work on those few SMS games, either put a switch in or cut the Pause button connection.

Ah I didn't know that, I've done it on an three button version of the Mega Drive Arcade Stick, which makes a great SMS controller.
