Worst game today: Heroes of the Lance. It's baffling that a piece of faeces like that got ported to a ton of systems. Did anybody really demand that?
Back in the day from those i owned i would say Running Battle, nothing positive to be found there: Flickering, bad controls, horrible colision detection, annoying flickery boss battles, the same level 3 times in a row with only minor changes plus mediocre music, mediocre graphics and horrible english in the cutscenes. I was so disappointed back then...

Worst game today: Heroes of the Lance. Quoted for truth. This for sure. Sadly, I live in the states and this wasnt available here ... so I imported it. What a mistake that was.

I'm making an effort the last few weeks to try some of the games I've had for a while and never played before. Honestly of the 130/140 games I have, maybe 50 have not been out of the box since their last owner many years ago. Just tried Strider for about 20 minutes. Worst game ever! I should have quit after 5 minutes. I may change my mind again as I give some of these old "classics" a try.
F-16 I guess is still the worst, with Strider a close second.

Back in the day the worst game I had was Indiana Jones and the Last Crusade. I got it as either a Birthday or Christmas gift and remember opening and thing wow this is going to be incredible! I couldn't even get past the first level, I hated how difficult and unenjoyable it was to play. I seem to remember my uncle (4 years older than me) getting to the 2nd stage on the train but thats about it. I grew up with the Atari ST version of this game. Hard as nails. A couple of years ago (21 years after first playing it), I finally completed the first level and started a thread on Atari Forum to commemorate the occasion. On the whole, I think it's a pretty good game.

F16 fighting falcon. Not fun at all. Clumsy controls, poor graphics (even for the day).

For me so far it's Superman Man of Steel. The cover art is amazing, one of the systems best but it's just pants.

I have a subquestion for this thread.
WHAT'S THE WORST SMS ARCADE CONVERSION:-
Well seeing as I chose My Hero as overall worst, which itself is an arcade conversion, then I've already answered my question.
Others unworthy of their originals:- Double Dragon Vigilante Alien Storm Space Gun

Yeah, I feel like half the games that people list on here I want to counter! Double Dragon was awesome because it was a) pretty faithful to the arcade map, and b) 2-player at a time when Nintendo had just laid a big egg with single player "Double" Dragon. Vigilante does have a famous exploit with the second boss fight that shouldn't have made it past QA, but aside from that it's also quite faithful to the original and is a graphical showcase for the SMS. Alien Storm I can't really speak to since I haven't played it, but I find it interesting because it's also a faithful adaptation, whereas the Gen/MD version was a re-imagining.

I remember getting Dragon Crystal back in the day, and had to judge my pick by the game descriptions on the box and the images there + what I've seen in the Sega catalogue. The cover looked awesome and the pictures were promising. I ended up returning it very soon after purchase though, because it was a TOTALLY different game compared to what I expected. I had thought it'd be something like GAW, but soon realized otherwise. Maybe it's not a bad game, but it was a disappointment in terms of what I had hoped for.

Ok, people are disagreeing with me on what makes a good arcade conversion. This is my idea of good arcade conversion:-
Fantasy Zone:-Good game, with all the essentials from the original. Captain Silver:-Possibly better than the original. Wonderboy 2: In Monsterland:-Great game, hardly inferior to the original. R-Type:-Again, barely inferior to the original. Rastan:-I was pretty satisfied with the downgrade.
